The Manual was created in Adobe PageMaker 6.5 and contains 310 pages designed to print back-to-back. Chapters and associated chapter forms are available for download/printing ONLY as individual PDF files. PDF files require the use of Adobe Acrobat Reader. A FREE copy of Acrobat Reader can be downloaded by clicking on the icon at the bottom of this page. Download/print only those chapters/forms in which you have an interest OR download (to your hard drive) all of the chapters/forms in a compressed format (MAC or PC). (See "Other Related Items" at the bottom of this page.) NOTE: Because the forms are PDF files, they are not editable. In order to customize these forms for your school/program, you need the Adobe PageMaker software application (version 6.5 or version 6.0).
